APPLICATION PROCEDURES

Each candidate should complete the application packet and submit it, along with a $100 non-refundable application fee, to:

• Application form
• Autobiography not to exceed five double-spaced pages
• Career history and professional goals statement including reasons for applying to CSWI. The statement should include what you hope to achieve in the program and your professional goals
• Summary of a treatment case, not to exceed four double-spaced typed pages
• Current Curriculum Vita
• Transcripts from both undergraduate and graduate school education sent directly from the educational institution
• Three letters of reference. One letter should relate explicitly to academic     achievement and the other two to clinical experience.Please ask the     references to mail letters directly to the Dean

In addition to the written materials, the application process involves personal interviews with two of the Institute Faculty members.

The deadline for submission of completed applications is the first of April for the academic year beginning in the fall. Applicants will be notified of the Admissions Committee decision within two weeks after the entire application process has been completed.
